wrong info in PIO card

my sons place of birth is i.e city is phoenix.it was correctly given in his US passport but in the PIO card city of birth was given as California.
will that make any problem while exiting US?
also what all documents need to be shown at the airports for US born kid(3 months old).US passport is enough?

I don't think that is a problem for traveling.

All you need is US passport and PIO card while traveling from US to India.
