PIO Card - Online form for SFO CGI

Hello,
I am planning to apply for a PIO card for my 4 month old and would like to know whether the application form can be entered online without having to fill it out by hand. The form at CGI SFO website is not editable. And the form at immihelp.com is editable but not the same as the one in CGI SFO website. Is it okay to use this one as opposed to the one in CGI website?

I am filling in the application and have a question on Q # 14. I am planning to show that applicant's parents (that is me and my wife) are born in India but they ask for a proof of residence in India. What proof should we submit? Is the Indian address in my wife's passport good enough? My passport was renewed in US, so it has US address.

Hi - the SFO PIO form has been updated on the SFO consoludate website (second page) - will your team be able to create a fillable form out of this latest form?
