Which visa for child born in US to Indian parents

We had a baby in US and now we want to go return back to India. Might be a few years before we come again on work. Which visa should I take for him? "Entry visa" seems to be the one but would like to confirm.

And don't I need to send copy of my passport with the visa application? As per the instructions they just need the US address proof.
