Sending Application
1. I thought I need to send part a, part b, check, all copies, my indian passport in first go but my friend told that just send part a/b & check once it gets approved then send passport & other copies !!! Is it true

3.I am applying for family of four. I can send only ONE cashiers check
$1120 (275 * 4 + 20) correct !!!
4.Do I need to send the self address as well as STAMPED envelope if so then why they charge $20?
Part A
5. I have 12 yr & 7 yr both can write so do I need their thumb or name will be ok?
Part B
6. Number 19 # Method of acuisition of Nationality ,
For US born child I need to mention the birthdate correct !!!
7. Number 20 # Previous Nationality
For US born child what should I write or do not mention her name !!
8 Number 21 # Address of Employer for Child !! What kind of question is that
9. Number 22# Details of family memebers, Just one is enough Correct !!
10. There are two spots for signature do me & my wife sign !!
11. I am parent / guardian ............. Do need to mention both kids names !!
12. Evidence of relationship as parent ......What should we need here !!
13. All the copies need to be attested (notarized) ?>?>
