Passport Renewal - Given Name(s) is currently empty

Hi everyone,

I have a question on Passport Renewal. In my passport the given name(s) is empty and the entire name is written in the Surname part in the passport. While I have to be renewing the passport when I see the online form the Given Name(s) field is mandatory and is represented with a *.

My question is if I take the print out having the given name(s) empty as it is in the Passport, is there any chance of the renewal getting denied?

Another question is, what should be the 2 addresses that I should be filling in the passport renewal form as in the above link which asks:

1) "Address to be Printed in Passport" and
2) "Other Address".

Should I be entering the USA address in the "Address to be Printed in Passport" and "Other Address" my Indian address or both of them Indian Addresses one permanent and the other my communication address in India.

What is to be entered in "Web File No:" which appears as mandatory field in clicking the "Re-Print Form"?
