Name Correction in Indian Passport

Hi,

This is YYYY who got the passport in the year 1999 and due to expire in the month of August 2009.

When I initially applied my passport, I did not have clear understanding of Surname and Given Names and hence, I would have made mistake of filling my First Name in Surname area by striking ‘Sur’ and Last Name in Given Area space.
So the passport was issued with the name as below,
Name ( ‘Sur’ is stiked in the passport ) : YYY
Given Name : ZZZ

In reality, it should have been as below,
Surname : YYY
Given Name : ZZZ

I did not have any problem until I got H1B Visa for USA, my company had filed my petition as given in the passport but mentioning
LAST NAME : ZZZ
FIRST NAME : YYY

So I would now like to correct my name in the passport along with extending the same. Could you please help me with the procedure to followed for this kind of case.
