Hi,




I am about to renew my passport as well as apply for a name change (together). I noticed for its $40.00 each for the service. My question is would it be $40.00 + $40.00 = $80.00 or would it be $40.00 for both of the services when done together.




Adding another question:
For a name change I saw I have to get this document "/indian-passport-services-in-usa/change-of-name-affidavit-indian-consulate-chicago.pdf" signed by Notary Public. In the form there is a sentence like this:

3. That I expressly authorize and request all persons in general, relative and friends in particular at all times hereafter to designate me, my wife, my children, remitter issue by such assumed name/surname of ________________________________________ accordingly.

4. In witness whereof I have hereunto subscribed my former and adopted name/surname of _____________ and ___________ office my signature and seal, if any, this _______ day of __________.

Out of the above two sentences, I wanted to know what should be filled in the blank spaces. What I think to be filled is as below:

First blank space: New/Modified Name
Second blank space: Old name
Third blank space: New/Modified Name
Fourth blank space: (Not sure what to be filled in here)??
Fifth blank space: Today's date

Could you please help with the right ones if the above are not right? And what should be the answer to the Fourth blank space?
