  • New York Consulate PIO Form Question

    I need to apply for my baby's (1 month old) PIO card to New York Consulate by mail. Their form has the following two lines:

    1. a) Name of the Applicant
    b) Surname at the birth (if any)

    My baby has a first name and a last name (which is same as my and my wife's last name). My baby does not have any middle name.

    My question is, what should I write on line 1 and what on line 2?

    1) Do I write the full name on line 1 (In which order? first last or last, first) and keep line 2 blank?
    2) Do I write only first name on line 1 and surname on line 2?

    Here's what I put on my PIO application for the Houston Consulate in 2008:

    1.
    a) firstname middlename surname
    b) surname

    PIO issued no problem.
