Change of Name - Married to Maiden

I am writing to inquire about the process for getting back my maiden name on my Indian passport. I am still married but am interested in retrieving my last name from before marriage.

- I was married 5 years ago in Mumbai while my husband was on H1B. On my marriage certificate, my maiden last name (TAWDE) was changed to my husbands family name.
- I got a new passport immediately after that which also reflected the new last name (HOLKAR).
- And then, the new name (HOLKAR) naturally got carried over to my Green Card.

Name before marriage: [My Name] [Fathers Name] [TAWDE]
Name after marriage/ Current Name: [My Name] [Husbands Name] [HOLKAR]

Desired Name: [My Name] [TAWDE] [HOLKAR] or [My Name] [HOLKAR] [TAWDE]
Visa Status: Green Card holder

I had a few questions regarding this situation:
1. Is such a name change possible?
2. If yes then what documents are necessary? Are there any special forms that I need to fill out other than the Passport Application Form and the Personal Particulars Form?
3. Once I have the passport with the new name, can I update the name on my Green card to match the passport?

I highly appreciate all your help in this matter.

PS: I have my husbands full consent with this name change.
