    Hello,
    simple question regarding the PIO form. i am applying at chicago consulate for my son. do they want the pio form printed on 1 page, or separate pages? the form has 2 sides, so just wondering. if i am going to mail it, i did not want it returned for such a reason, but you never know with the indian consulate, so just wanted to check from folks who have already done this.

    Also, for photos, is it fine to take the passport photos at Walgreens, for example?

    I sent mine into the Houston Consulate on two pages, no problems.

    Passport photos from Walgreens, Fedex Kinkos or anywhere are OK so long as they're the right size, etc.

      thank you for your response. Just wanted to update.

      Application sent in on Oct 6, received on Oct 8 in Chicago consulate.
      Received PIO card today Oct 20.

      please note that they have updated the supporting documents list on the website and the form recently. (for anyone just going to apply)
