PIO Card - Online form for Washington DC

Hello,
I am planning to apply for a PIO card for my 1.5 month old boy and would like to know whether the application form can be entered online without having to fill it out by hand. The form at website, Embassy of India-Washington DC is not editable. And the form at immihelp.com is editable but not the same as the one in Embassy of India-Washington DC website. Could you please suggest me which one should I use, or if you have the fillable version of the latest one available?

Font Size too small for certain items

In the new fillable form (thanks to Immihelp for making it), certain items have very tiny font size, e.g. item numbers: 1(e), 3(a)(b), 11(a)(b), 12, 13, 14, 15, 17(c)(d).
is it possible to increase the font size of these itesm?

There are few still there:
11(d): yyyy coulmn
11(e)

and if possible (not too much of trouble)
4(d): email (though, the space is very limited)
in item 8, the box is off-centered so doesn't get highlighted fully, but that is not a major issue.
in item 13 and 14, if you click YES or NO in one the other item's YES or NO also gets clicked.
