Oci Application -- Blank Pages Printed

Hi Friends

I am a us citezen and live in New York. I have applied for oci online and at the end when printed i got only one page filled with the data that i entered online and rest of the pages are blank. I am using internet explorer. Does this happend to anyone? Please help.
