PIO card application for San Francisco

I am submitting PIO card application for my son, where both parents are US Green Card holder and citizens of India.

Question 1:
Form field #13 has my info, Should/Could #14 carry my details as well or should it be N/A.

Question 2:
When applying in person, does it require both parents ?

Question 3:
From the eligibility criteria of OCI, it doesn't look like that's an option, since both parents are Indian citizen. Is this correct ?

Comment:
Fill-able Application Form has some low height fields.
e.g.
Date fields: 1e, 10c, 10d,
Phone : 4a-4c
Text Fields: 13a-13c, 14b-c, 17 (Declaration: name field for both parents)

These fields are barely readable on an inkjet print. Fillable fields could stretch below the line or above into printed area, as there is some offset on top and bottom.

Are you talking about the problems with the form at https:///us-visa-and-immigration-for...-francisco.pdf ? If so, we just modified it to make it better. Please try it again.

Form looks better - My questions are still open

Thanks for replying. Form now looks lot better and is readable when printed on laser printer.

Fields 1e, 4, 10 and 13c should be increased in size. Looking at a print of filled form will show the problem.

I am filling the form with those fields blank. These are then filled in by pen after taking a print-out.

It's still a big help as most of the form can be filled on the computer.

Please help in finding answers to my other questions.
