I am on a h1-b and am filing a passport renewal to the san francisco office by mail.
I wanted some help with some of the fields in the applications form
1) On page 1, right at the top they ask for amount paid. Does this include the amount I paid for the self-addressed envelope or is it just the fees for the passport renewal.
2) Field 12 asks "how long has applicant continuously resided abroad" . Is this just the amount of time I have been in the USA since I first came from India or is it the amount of time on my current visa or is it actual time I have continuously stayed in the country. If it is the last option, then any trips to canada would break the continuity.
3) Field 19 asks for name and address of two relatives/friends. Does it matter if these relatives/friends are in india or the US?
4) Field 24 asks "is applicant registereed with indian mission/post"? What does this mean?
