I would like to know the documents to be send for Minor - Indian Passport Renewal.
A) Do we need to send our (Parents) original passport too?.
B) I dont have any Proof of address for my daughter as address is in my name. What should we do for this?
Is the below list correct one. Becuase i am seeing so many documents to be notarized.
1) Prev passport
2) First 5/Last 2/ and Visa page of Passport
3) I - 94 (Notarized)
4) Proof Of Residence - Water bill, Electric bill(Notarized)
5) Passport Photo 3.5 CM * 3.5 CM (Front Facial Mode Color, White background)
6) Change of appearance form (Notarized)
7) Photo notarized on application (signing on front and pasting on last page application) (Notarized)
8) Birth certificate (Notarized)
9) Original passports of both mother and father
10) Attested copies of the first five and last two pages of both mother's and father's passports
11) Consent of BOTH parents (on the last page, page 4) of the online printed application both parents should sign and have the signatures notarized (Notarized)
