I want to apply for PIO card for my US born daughter in SFO consulate, following are the documents I am planning to carry with me, are they sufficient or do I have to take anything else apart from these.

    - Application form filled in duplicate.
    - Original & photocopy of US passport of my daughter
    - Original & photocopy of birth certificate of my daughter
    - 2 photos of my daughter
    - Original & photocopy of both parents Indian passport
    - Original & photocopy of Utility bill for address proof.
    - Planning to pay fee using debit card.

Do I have to take an appointment at consulate for apply PIO card in-person.
Since I am going to carry originals, i am not planning to get any of the documents notarized.

Your help would be highly appreciated.
    Thanks,

From the bottom of the SF Consulate's PIO page (emphasis added):
Fees and mailing charges should be paid through Money order or cashiers’ cheque drawn in favour of the Consulate General of India, San Francisco. Personal cheques, credit cards or other banking instruments are not accepted.

Just go to your local Safeway or whatever and buy a money order with your debit card -- the fee will be ~$1.

They don't have appointments, but only accept submissions between 9am - 12pm. It tends to get quite busy, so I suggest arriving before opening time.

      Thanks Curtdfw,

I applied for PIO, with list of documents i mentioned in my earlier email and with money order as suggested by you.

Along with, list mentioned in earlier email, I got declaration in Application Form notarized, as both parents didn't travel to consulate.

      Thanks,
      Aveer Jain
