Indian passport renewal and change of address

Hello,

I am looking for some immediate help regarding indian passport renewal
and change of address at the NY CGI.

I am in US for the past 5 years. The Indian address on my passport is
not valid any more. To prove the new address in India, I have the
ration card (not in English, with all the family members on it) and
electricity/telephone bills (at the new address but not on my name,
they are on my father's name). Will this work?

Can I apply for passport renewal and change of address at the same time?
The nationality verification status form needs Permanent Address in
India along with two relatives/friends. I cannot write my new address
unless it shows on the passport as well. Am I right? If I keep the
same address as in my current passport as my permanant address, then I
cannot show anyone at that address. Tricky situation.

I went through all the guidelines on the website but I am still
unclear. I have to get this done as quickly as possible since its time
for my visa extension.
