Applying at Mumbai FRRO

Hi, I am in india on PIO and will be applying at mumbai FRRO soon. My questions are....
1. Do I need to submit just the form in triplicate or all the photocopies too?
2. What is the amount I need to get a demand draft for, (in INR as I will be applying in india and has a PIO)?
