Procedure for Mailing US Passport for U-Visa (OCI) Stamp (NYC)

The NYC consulate page does not have detailed instructions on how to mail my US passport for the U-Visa (OCI) stamp. My OCI application status shows as "Documents dispatched to concerned mission/office". The last step for me is to get my US passport stamped. However, I live in Massachusetts, and prefer to mail my passport in for the stamp. Does anyone have detailed instructions on how to send in the US passport for stamping? Does the Indian passport need to be sent in for cancellation too? Thanks.

Thanks. The page that you reference has instructions for mailing to the San Francisco consulate. Are the instructions the same for NYC (that is, for the cover letter, and the header on the envelope)? Also, should I include a self-addressed Express Mail envelope?
