Indian Passport Renewal - Spouse last name is missing

Hello,

I have to renew my passport as it is expiring in June 2020. There is no Parents and Spouse last name in my current passport .

For parents, i see Name change option in Miscellaneous services in CKGS website but no similar option for Spouse instead add/delete spouse name.

Can anyone let me know what option to choose or is it okay to have spouse name w/o last name?

Here is what I would do - put in the full name in the place for name, first name, middle name, last name, everything as you would write her full name. Match with what is in their passport.
