  • Getting name 'Split' in passport.

    Hi,

    My wife's passport is due for renewal and will need to get it done from the Consulate General in Houston. I would also like to get her name 'split' appropriately in the new passport, as both her firstname and lastname are given together as Given Name in the current passport.

    Does anybody have experience in getting this done from the consulate at Houston? I could see a provision under 'Misc.Services' for New York Consulate, but dont see that option in the website of the Houston consulate.

    Can someplease let me know if they have an experience on this? Can you let me know what's required/needed to get this done?

    Would appreciate suggestions on this.Thanks in advance!

      When you fill the passport renewal form, fill in correctly with given name, and surname in the corresponding fields. Do not write the whole name in the given name field. Then include a notarized copy of Name Change affidavit, and a letter explaining you would like to split the name. You should be good to go!
