    Perhaps someone can help. I have lost my Indian passport not sure what happened. I don't even have a copy so I have nothing at all. I have my first passport that was issued in india.
    So I send OCI application with the following documents

    Details of family members
    US driver license.
    Self attest copy of first two and last two pages of US passpor
    Self attest copy of the first three pages and last two page of my first Indian passport.
    Domicile Certificate.
    Birth Certificate.
    Bachelor degree certificate issued by University of Bombay.
    Post-Graduate degree certificate issued by Sardar Patel University.
    US Naturalization certificate.
    Affidavit for lost Indian passport.

    I get the application back now they want
    1. Color photocopy of green card - all I have is B/w copy of the green which I had to surrender since I got US citizen, in case copy of green card is not available then orginal I-130 and supporting documents, wel I got my GC in 1992 and all the document are with the lawyer and/or with company that sponsored me.
    2. Copy if long term status in US such green card/H1B/F1 visa.

    A per the website if I don't have the orginal passport I need a affidavit which I send explaining how it was lost and that I did not hold any other nationality other then India and US and any two of the following.

    copy of domicile certificate.
    copy of any photo id issued by gov of india - voter registration etc etc.
    Copy of birth certificate and copy of passport of parents
    copy of indian educational certificate
    copy of indian land ownership.

    I had send domicile certificate and copy of graduate and post-graduate certificate. I am not sure how to deal with the SFO consulate office. I am going to attempt to send my application with B/W green card, approved H1b visa and passport and PAN card copy of my mother and hope that will work.
    Any pointers if should do anything else?.

    this gets more frustrating. I send e-mail asking for clarification and I get reply stating not to send in since the requirements are changing....argggggh.
    Does anyone know or have recently send in applications for OCI?
