Can anyone tell me what its means the forms to be filled in duplicate?
Can I apply for surrender certificate in person in NY? Will I get the certificate and the passport cancelled at the same time?

Means two copies of the application needs to be filled in. Both needs to be notarized if you are not in person visiting the consulate.

Yes you can apply for S.C in person in NY.

Not sure about the third question. Considering it is june, you might already figured out the answer.

I got mine today, yes you can apply in person, the website clearly mentions that. I got it in 10 mins, but people who did it yesterday got it in the evening same day. So I think its just your luck, the dya you land up in the consulate.

Hi,

since you got the SC I am sending email to you. The SFO consulate today sent me back my passport along with the SC application form with a stamp affixed to the bottom of the form. They did not send any separate certificate.

They also took the $175 application fee.

Did they actually send you any separate certificate?
