Urgent- Personal particulars Req'd

Hi,

I renewed my passport in India for spouse name addition through thatkal & came USA with my new passport but it seems address verification would be done later for thatkal passports. I was not available at my indian residence when came for address verification & now I am asked to get a personal particulars endorsed from Indian embassy in Houston. Could anyone please tell me on what needs to be taken to the embassy for getting this personal particulars form?
