New OCI Application Process and Documents List as of June 12 2020

I started filling a new OCI application and unable to identify the list of documents that needs to be upload on the government website. All old videos and sample present on the internet looks old and does not match with current government website pages.

I am looking for list of documents to be uploaded on govt website as first step for minor born in USA in Jan 2020 with both parents are Indian nationals. While filling the form there is a pop up coming up which says I need to upload a parental authorization form which should be notarized.

So confusing. Please help.

I am also filing fresh OCI application for my daughter [minor], so far i have completed the PartA and PartB on Govt website
Here is what i have uploaded on their website and this gets printed on the OCI Application Form after you print it.
1) CURRENT PASSPORT
2) CURRENT INDIAN PASSPORT OF PARENTS/GRAND PARENTS/GREAT GRAND PARENTS/SPOUSE
3) ADDRESS PROOF
4) PARENTAL AUTHORIZATION (IN CASE OF MINOR APPLICANTS) 5) LEGAL STATUS OF PARENTS (IN CASE
OF MINOR APPLICANTS)
6) RELATIONSHIP CERTIFICATE

You will get the Parent Auth Form on CKGS website under My Account.
Use the "USAS..." reference number from the Govt website and create new oci application on CKGS website once you complete that you will endup on the final page which will give you the Parental Auth Form.
You can hand fill, notarize it and upload on Govt website.
Print the latest OCI Application so that it reflects updated documents

The portion where i am stuck is on the CKGS website where it says they need
1 - Affidavit in lieu of Original - i cannot find this document anywhere...links are broken on CKGS and Indian Embassy US
2 - Additional Particulars Form
3 - Not sure if i have to send my minors Original Passport in the courier now or is it only needed later during the matchup.

Its been a week since i emailed CKGS and still waiting for some direction from them. If you have emailed them and have better clarity then please let me know as well

Thank you for your response. So basically:

1) Current Passport - for my daughter
2) Current Indian Passport - Both Parents passport to be combined in single pdf.
3) Address Proof - What should I give for this?
4) Parental Authorization form - I will get from CKGS
5) Legal Status of Parents - I will put I797 C as my H1B extension is in progress.
5) Relationship Certificate - Birth Certificate

Hi,

This is mentioned in the ckgs site, It is mandatory to upload all the necessary documents online on the OCI Government of India Website before you take the print out of the OCI application form to avoid technical rejection. So do we need to go to the notary twice, first for getting the parental authorization form notarized and second after taking the print out of the application form. Please let me know?

Also where you able to find the Affidavit in lieu of Original ?

Did you get the PARENTAL AUTHORIZATION form notarized before you uploaded into the OCI website? Please can you tell me

salem Yes i have notarized my Parental Auth form before uploading on Govt OCI website. But they have a option to re-upload documents so that should allow you to upload another updated copy later before the final submit.
After you upload this document on Govt website, you can reprint the application using the left column bottom "Reprint application" button. After all documents are uploaded and i have printed that OCI application i will notarize it as well.

Affidavit in lieu of Original - This is still a misery which is how i landed on this website

Got a response from CKGS

" The applicants apply for the OCI card under the 'New OCI' category does not have to submit the 'Affidavit in lieu of original & the Additional Particulars form'.
Also, we request you to follow the document checklist."

That takes care of my 2 questions, i dint hear back if original passports need to be also sent - but since the checklist did say Original and Copy i will be inclined towards sending original as well.
Will update this on my thread as well for reference

ArthurD After taking print out of OCI application form, Page 3 shows only Applicant signature in case of minor and that will be thumb impression, where parents have to do signature and at which place need to notarize. I am not able to find any place. Can you please help.
