Passport renewal query

Hello,

I started my application to renew passport through CKGS website .

Currently I am on step 3 (Forms). This page tells me to review Affidavit for change in Appearance/signature. Page display is as follows:

I (my name displayed) son/daughter/wife/husband of (name which is blank), resident of (usa) and holder of passport no(displayed) issued on date and location (displayed).

When I click submit. I get this error message "Please provide appropriate name through NVF ".

Note: I selected " No for both change in appearance/ signature".

I am not able to proceed further. I have emailed CKGS but no response received.

Anyone encounter this issue before ? Do I need to start all over again ? Kindly share your valuable input . Thank you !
