    My daughter is a minor. Her existing OCI card has wrong date of birth (day and month got switched). How to get that fixed and also get it re-issued for a new passport? Can they be done at the same time?

    What documents do I need to upload at the govt website for this?

    Tried emailing CKGS but no reply. When I call their phone number, it says unallocated.

    Can someone please help?

    I think you will need to pay the fees again if it was an error on your part. They won't fix it for free. Are you talking about her US passport and that has incorrect DOB format also?

      US passport has the correct DOB. But the OCI card which was issued several years back has the wrong DOB. She just got a new US passport. So we need to get OCI for the new passport as well as get the DOB fixed on the OCI.

      Anyone knows how to contact CKGS Chicago office?

        Apply for OCI Miscellaneous Services since she already possess an OCI card that needs re-issued (+dob correction)
        Since this is a month and date mix up, you can always claim you never realized it due to oversight.
        Visit https://ociservices.gov.in/welcome and click on "miscellaneous services". You need to complete this formality first before applying on CKGS
        I would include a notarized letter that explains why you are seeking a DOB correction along with the re-issue.
        When submitting documents, include a sticky note on the bio page of the existing OCI card that says "birth date has month/date switched and needs corrected". This way, you are covering all bases.

          Thanks everyone for your response. I was told from the consulate to select two options: (i) change in passport and (ii) change in personal particulars.
