Legal status document for passport renewal

I am in the process of applying for my passport renewal at CKGS-Houston.
The process requires submission of “legal status in US” document. The options provided are following:

1. Green Card
2. EAD
3. Valid I-797
4. Valid I-20
5. Valid dependent visa
6. B1/B2

I am on a valid blanket L1 visa. I am not sure which document i should send. I understand that the category I need to select is Valid I-797 (Option 3). However my I-797 is on company name and not on my name.
Can I send a copy of my Visa or approved I-129S.


Request your help to kindly clarify which document is required to be submitted for Blanket L1 Visa holders.

You should send the 797 copy + Visa copy + original passport.

The requirement says that I-797 needs to be notarized, however i do not have the original copy. Should i send plain color copy of I-797 with notarized visa copy?

Proof of your legal status in the US needs to be "true copy - notarized". Only select notaries can do this and they will need to see the original document.

You can try going to the bank and getting a copy notarized. Since you already have a visa stamp, I do not see this being an issue in your case.

Ashish321 Were you able to get a color copy of I-797 from your employer? I am in the same situation (L1A blanket visa). When I asked a color copy of the I-797 from my employer, they said that the original I-797 for blanket petitions is black & white only. Kindly let me know if you got a color copy of the blanket I-797 from your employer.
