How to get the affidavit from Indian embassy at USA

I am currently located at pittsburgh, USA for job and my wife and newly borned baby boy are located at Pune, India.
May I know, how to get the affidavit from Indian embassy at USA as My wife needs to apply for passport for my baby boy at India.

You need to send a notarized affidavit to the Consulate at New York along with your passport, a return mailer and a $10 money order
