Passport renewal, Mismatch between current passport and Government Passport Preview

Hi,

My current Indian passport is expiring soon, so I wanted to renew it with SFO consulate. However I see there are couple of mismatch between my current passport details and the Passport Preview Details page (after completion of Government form, before submitting)

Place of Birth:
Current Passport: Mumbai MS (I think this came from my earlier expired Indian passport, which read as Mumbai M S, which probably stands for Mumbai Maharashtra State)

Government Form Entry:
Mandatory: City/Village/Town: Mumbai MS
Mandatory State: Maharashtra
Mandatory District: Mumbai

Resulting preview:
Place of Birth: Mumbai MS, Maharashtra (Mismatch from current passport which reads as Mumbai MS)

Name of Father and Name of Mother:
Current Passport: LastName FirstName
Government Passport Preview: FirstName LastName

Did anyone else have similar experience with the Government form and its Preview not matching existing passport. Not sure how to make them exactly same as Current passport to avoid problems with CKGS / Consulate.
