Renunciation Procedure using VFS

Hi, I am trying to fill up this Renunciation form - https://visa.vfsglobal.com/one-pager...lable-form.pdf
The dates are in MM/DD/YY format as specified by the pdf, whereas in other forms the dates are to be entered in DD/MM/YYYY format. What date format are you using? Somewhere in the checklist I read that we are not supposed to handwrite anything, so I am not sure if I use MM/DD/YY format, would it still work or not?

But this clearly says in the title it is for Naturalized On or After 1st June 2020. But I was Naturalized before. So do I need to fill this checklist form? Is there a different checklist for before 1st June 2020?
I also saw a step where it sent me to register on the VFS website, which I did.
I am unsure what I need to do next. Any help with the steps will be much appreciated.

Wondering if anyone can answer this question as well. My mother was naturalized before June 2010 and we don't have her Indian passport, so which checklist are we following? There's only a checklist for naturalization after June 2010 and I don't see any checklist for before 2010 anywhere. It's super confusing. I'm about to send the application with the 'After June 2010 checklist' just in case.

If it's showing same "no data found" for quite a few days, you may ask them otherwise may be just wait and watch.

2 quick questions:
1) What is passport number that needs to be entered in the VFS questionnaire - Indian or US passport? And about the passport number at the top of the checklist?
2) How many photos are needed in all? One needs to be pasted on the government application. Do we send 1 more photo or 2 more photos with the application.

Looks like the Renanucaiton of Indian Passport is the most difficult of all the applications with Indian Consolate. Worst documentation and handling of the Process by VFS.
Did anyone got there Indian Passport renanuced after VFS took over? Any Success story ??
I have filled the application of surrender of Indian Passport on Govt of India Website. That seems ok. Just that the digital photo upload and signature option is not available
I assumed thats not needed.
I have question on the VFS Global site. After registering and logging into the VFS website, The Create New Application form the first page is ok. Filled it . In the Add Customer form are these details of Indian Passport or US Passport ?? -
Passport Number:
Passport Expiry Date:
Select Nationality* : (This I believe we have to select US as there is no option for India)
Expiration Date of most recent Indian Passport* : this seems like Indian Passport expiration
Why 2 expiry Date? do we have to put both same.
Also is the online CreditCard Payment working ?
