    Once we upload the mandatory documents in pdf format we get the final form from government of India website. That form has a declaration page which says:

    DECLARATION

    1. (Name of the applicant) .......................................................... solemnly and sincerely declare that
    foregoing particulars stated on this application are true and I make this solemn declaration consciously
    believing the same to be true. In case the information is found to be false at any stage, I am liable for
    legal action/cancellation of registration as Overseas Citizen of India cardholder/deportation/blacklisting
    or any other action as deemed fit by the Government of India. I have no objection to share the
    requested personal particulars for processing of Overseas Citizen of India card by the security
    agencies.
    2. I shall not use the Overseas Citizen of India card for undertaking any TABLIGH activities in India
    3. I shall not use the Overseas Citizen of India card for undertaking any missionary or mountaineering
    activities or Research in India without prior permission from the competent authority designated by the
    Government of India
    4. In the event of dissolution of marriage or marriage to another foreigner or death of Indian spouse, I
    shall surrender my card to the issuing authority or mission [ applicable to spouse based applications].

    Now I am applying on behalf of my 4 month old son so should I:
    1. Place my son's left thumb impression in the part which is after "*Application for Minor Child" OR on both Point 4 before "*Application for Minor Child" and after "*Application for Minor Child"
    2. Do me and wife has to sign this document? If YES, then where?
    3. On which page number the notary should place his stamp?

    Hello ms1989,

    In the DECLARATION section, the name of the applicant would be your Son's name - In the Signature of the applicant section you sign (Parent).

    In below section i.e, the section for Application for Minor child, both parents have to sign, ( me and my wife signed). This signature has to be notarized, in California notary is a separate form so you just attach the notary page/form to this application.

      Do we need to self attest the registration form alongwith all the supporting documents? Just trying to understand where self attestation needs to be done..

        chakra55 Hi,
        In the form for minor application there is a placeholder for one parent to sign. How did you and your wife sign? Below the placeholder?
        one more confirmation, as per your comment you didn't get the Part-B notarized but just the separate form. Please confirm
