OCI- Minor application

Hi All,


I'm planning to apply for OCI for my month daughter. My wife and I are Indian citizens having H1 and H4 Visa.

Registered on Govt. OCI website, I will upload these documents online

1. My child Passport

2. Marriage certificate

3. Parents Indian Passport

I will send the following documents to Chicago Consulate/VFS.

1. My child Passport copy

2. Marriage certificate

3. Parents Indian Passport copies

4. Address Proof -DL copies

5. Parental authorization form - Notarized

• What should I write in the Capacity option- Is it Parents?

6. OCI application form with one photograph

7. VFS Checklist signed copy

8. My Daughter's US passport


Am I missing anything? Which other forms should I notarized /Include?
