OCI - Few Questions

Hello, today I received my renunciation certificate which took 5 weeks. I applied in SF consulate.

I started application process for OCI and have few questions-

1) My mother's name is slightly different in what is indicated in my Indian passport vs what is indicated in my Mom's passport. So my question is which name I should indicate in the application?
2) National Identification Number - I read in the forum to keep it blank.
3) I have selected "SELF" for "Relation with the Root Indian". Just wanted to confirm.

1) Just use the one in your indian passport. You will just be submitting a copy of your passport for OCI for proof of indian origin.
2) This is not applicable
3) Yes, Self is correct.

Previous is your most Indian Passport that was cancelled.

I just finished the VFS application and paid online with roundtrip courier ($30) + $15 for returning passport but I print the courier label, it printed only one to send the application to VFS. Should it be printing three labels? For renunciation certificate, it printed two so I am confused now.

I have the same issue . I remember reading somewhere they are printing the labels them self now a days but not able to find that link...

I read below post and as per the OP, seems like policy change to print only one label, VFS will hold the other two labels for return!!! This is so confusing. I will go ahead and mail my application, hoping no glitches given the receipt clearly shows they charged me $45 for three lables.

Hello, another question - the check indicates to affix photo "The physical photo must be affixed on the physical application form and the digital photo must be uploaded on Govt. form online. The physical and uploaded photograph must be identical" but in the OCI application form there is no place to affix it. I see the digital photo & signature is printed. Where to affix it?

Ignore that and just send lose photos with the application.

Filling the OCI application on the Govt Website. In the document upload section. there is option as Employment/Work Letter ? is that really required. Do we need to upload the Employment letter?
Mine is first time OCI application after Naturalization. I have got my Indian Passpost renanciation done.
Any inputs?

When filling OCI application, I saw popup indicating below docs are required, I uploaded below as well as copy of my US passport, Cancelled Indian Passport and Marriage Certificate. I sent the application today for me and my wife.

A. Naturalization certificate of the applicant's current citizenship
B. Renunciation certificate of Indian citizenship
C. Address proof

Filling the OCI application on the Govt Website. In the document upload section. there is option as Employment/Work Letter ? is that really required. Do we need to upload the Employment letter? I never did. Didn't have any problems.

is the OCI Getting printed in Indian Embassy's in US or is it going all the way to india to get printed ?
