Appling for new OCI card for my minor child.
as per new checklist available in the website, do we need to notorize all pages of child USA passport pages copies or any specific pages.
Also please confirm whether we need to self-attest all xerox copies pages we need to send?
Also please confirm what documents need to be noterized.. Parents authorization form, child USA passport and any other documents. Heard we need to notarize last page of application form. DO we need to notarize that one also?

You have to send the original US passport, so don't know why they still ask for notarized copy too. No rhyme or reason, just blind bureaucracy.

Anyway, you need to notarize two pages.

1. Information page that contains the picture and personal details of the holder of the Passport.
2. Endorsement/amendment pages.

Also for most states, the concept of "notarizing a photocopy" does not exist! They should have said, notarize the self-attestation signature on the photocopy of your passport. Also, for minor's I am assuming that that the parents sign the child's passport photocopy and the notary fills in the Notary Acknowledgment attesting those parent's signature.

Most notaries will refuse to notarize this photocopy as it is against their legal binds. I know many notaries in Seattle/Bay area, who have refused to do this before. This is just pure BS. I feel for you, try to ask around in your city.

No one will notarize copies of license/passports. In CA I had experienced this and the agent refuse to notarize it. However they will notarize a statement.
You can say something like " This is a true and correct copy of my U.S. Passport.” then they'll notarize that statement. I had recent experience as lpr card and dl copy notarized was reqd.

Thank you for the information. Should i take a color copy of the passport and leave space at the bottom of the page and write the statement that you have mentioned and ask the notary to attest that?

Hi, I am starting the OCI application for minor child as well. Wanted to check in which other documents needed notarization besides passport and parental authorization form. Do we need to get the last page of the government application form notarized as well? we already have the parental authorization notarized. seems like duplication. please could you update on your experience. thank you.
