New OCI -Documents to be uploaded for Minor

Can anyone clarify what are the documents have you uploaded in the https://ociservices.gov.in/ website. It is not very clear.

I and my wife are currently on H1-B. This is what I am planning to upload. Please let me know if additional documents are needed


Current Passport - Passport (only photo page) of minor
Marriage Certificated - Marriage certificate of Parents
Address Proof - DMV of Father and Mother attached
Legal status of Visa - I 797 of father and mother along with Passports of father and mother
Relationship certificate - Birth certificate of minor
Parental authorization form - authorization form notarized
