Today I received email from VFS SFO that I need to send notarized AFFIDAVIT IN LIEU OF ORIGINALS which I somehow missed and have below questions -
1) I assume I need to indicate my Son's name here.. He is 15 years old
I [MY SON'S NAME], solemnly swear that the enclosed documents are true copies of original support documents submitted along with OCI application:
2) List of documents to list: The checklist shows only US Passport but what other documents needs to be listed?
a) CURRENT PASSPORT
b) ADDRESS PROOF - DMV of Both Parents
c) LEGAL STATUS OF PARENTS (IN CASE OF MINOR APPLICANTS)
d) PIO CARD - The card is in possession of VFS so I really can't list in the doc.
e) RELATIONSHIP CERTIFICATE - Birth Certificate
f) PREVIOUS INDIAN PASSPORT OF PARENTS/GRAND PARENTS/GREAT GRAND PARENTS
3) Do I need to indicate both Parent's name under this ?
Subscribed and sworn to by [BOTH PARENTS NAME] before me on the [ ] Day of ,
4) Signature: I assume both parents needs to sign in front of notary?
