  • confused about new OCI for minor

I have a few questions regarding the documents checklist. It mentions that the Gov application form requires parents signatures and notary. However, there is no place for both parents signatures and notary on the gov application printout. Also the signature line says "Signature of Applicant" and not signature of parents. Who is supposed to sign on the last line?

One more question, does the child signature also need to be notarized or just the parents?

The notary is usually a separate document that is attached with the other forms. You don't nee space for notary in the form itself. Take it to a authorized notary person, they'll know what to do
