New OCI Application - Question on Photo affix

I am filling out new OCI for myself (adult) and I have these questions. Any experts help will be of great help.

I am following the above check list for the documents to be sent.

Should we attach the passport size photo to the physical application on top of the digital photo? That is the only place that I can see space.

Also, it indicates that we only need to notarize US passport. Are there any additional documents that need to be notarized.

Yes, just attach the photos to the application form with a paper clip. Do not glue them.

And yes, only the US passport copy is to be notarized. Don't know why they ask for that, since you have to the send the original passport anyway - no logic, just blind process.

As of Feb'2023 - US passport copy does NOT need to be notarized, and original US passport is NOT required to be sent.
