New OCI for Minor - Which one is the checklist to follow from VFS?

It says we need to sign and send checklist along with application. Which checklist is "New"? I am so confused with document requirements

I found following checklists with somewhat different requirements. Has someone created a working one? mind sharing, pls? thanks!

The first one, the one that is named "oci-checklist-minor-final.pdf"
If you want to be certain, email VFS support and they will reply in 2-3 days.

The 3rd one is a sample form showing how to fill the application.

The 2nd and 4th ones are older versions. I wish they cleaned up their site and kept only one version. If you are feeling adventurous, go ahead, compare the three and note the differences ????
