OCI in Lieu of Valid PIO for Minor

Hello - I am applying OCI for my daughter in Lieu of valid PIO. Couple of questions:

- For proof of address - Do I need to submit both my daughter's birth certificate and my license as address proof..?.My daughters birth certificate address is different with my current address.

- Relationship Certificate - In docs checklist, they are requesting this document as mandatory to upload in govt website. What doc should we submit in this scenario.
They didn't mentioned anything about birth certificate.

What all docs need to be affidavited in lieu of original docs..
