  • OCI Card Minor- documents to notarize

    Hi, I am starting a OCI application for minor child. Wanted to check in which other documents need notarization besides passport and parental authorization form. Do we need to get the last page of the government application form notarized as well? we already have the parental authorization form notarized. seems like duplication. please could you share on your experiences. thank you.

    Yes, last page of Govt application form has to be notarized. See the attached screenshot

      Hi,

      In Proof of Address I see below, seems one more document to be notarized.

      - In case of applicant residing with parents/children/ relatives, a notarized undertaking from them with one of the copies of address proof as indicated above.

      Where can we get undertaking form?

        raj_hpking that means an adult applicant who does not have address proof of their own. That does not apply to minor applicants. For minor, address proof of parents is sufficient. No need for notarized undertaking..

          That is strange, why vfs mentioned it in minor checklist then. Very confusing.

          Also, do you know if my son has to thumb print on US passport for notary or I can go to notary and provide my sign.

            Yes, VFS instructions are confusing at each and every place. Your son doesn't have to put a thumb print on passport copy. You don't have to sign on passport copy either. Only the notary will stamp and sign it. I went to my bank where I have account to notarize. They notarized OCI application last page and parental auth for free. They directed me to a State Farm insurance office whose agent notarized passport copies for free!

              thanks, that is really helpful reply. So at notary they won't ask to sign or thumb print for my son? I thought for notary he or parent has to sign. I went to BOA earlier and they mentioned they won't do notary for passport and oci services. so doing now at UPS store.

                Hi.. I went to chase to notarize the parental authorition form and he did not stamp the main document and instead gave me an notarized form which says verification on oath or affirmation with affiance statement.

                Will this work or do I specifically have to get a stamp on the main form?

                  Hello,

                  I had the same issue yesterday and so I created the parental authorization form that matches the VFS format and the bank's notary section format. You may use the one attached here. Good luck!

                    do we have to notarize minor passport? checklist says only self attested photocopy. Indian embassy says only parent signature is enough for minor oci application.

                      • Is it necessary to notarize minor passport (the checklist only says self attested copy)
                      • Did anyone have problem or success with a certificate from notary attached but not on the same page as the parental consent?

                        Hi,
                        Need one help on Minor applicants photo and thumb Print. I filled the govt form online and uploaded photo with thumb print. Now when I printed that application photo and thumb print already printed to form but as per the VFS checklist we have affix the Photo and thumbprint on the printed form. Any advice do we have still affix the PHOTO even if it is printed on Govt Form?

                          I am just starting my daughter OCI application.
                          on website we need to upload the supported document.

                          do we need to notaries this documents then upload and send same copies ?.
                          or completed the online forms with documents then when we send in mail do the notary?.

                          please let me know.

                          this are the support documents

                          1. Kid USA Passport
                          2. Relationship doc - Kid USA birth certificate
                          3. Indian origin - Parents indian passports together in one pdf
                          4. Pic
                          5. kid thump pic.

                              did you notarize your child's passport copy? I'm also confused if I need to send a notarized copy, rather than just self attested(with parents signature).
