OCI Query : parental authorization form for minor

Hi,
I submitted OCI parental authorization form : https://www.vfsglobal.com/one-pager/...orm-minors.pdf
and I submitted with notary but I got an email that "The parental authorization form for minor that you submitted in not for OCI. Submit a notarized parental authorization form for minor that is specifically for OCI." Can someone please redirect me to the correct form, I couldn't find it online.

sharath64 There are separate parental authorization form for renunciation and OCI. Are you sure you used the 1 that mentions OCI. I had initially made the same mistake and printed the wrong parent auth form but I was lucky to see the fine print and then got the OCI one. See the attached screenshots below.
