OCI for Minor asking for Letter to Embassy to update given name in Government Form

I filed for my childs OCI and the application is on hold. They are requesting that I write a letter to the embassy to update the given name. The passport, birth certificate and all documents names match. Why would the request this letter? Can any one provide guidance or a template for the letter for me? I called VFS and they stated that all it had to say was that the applicants name and the name on the documents filed are for person [First_Name] [Last_Name].
Also we filed on the basis of the root indian being the mother and they are asking for a copy of the Fathers (me) Indian passport, and stating that it is required.
