Have a question that hopefully has been answered before since quite a few applicants would have faced this situation.
I am in the process of submitting a 1st time New OCI application for my wife. Its an independent application since she also was an Indian citizen before taking US citizenship.
Her last name/surame change happened right after marriage and all her recent Indian passports have had the new name since the marriage happened more than 2 decades ago.
In the application form, should I state the Name change as No and leave Previous name fields as blank, because the new surname has been in effect for 20+ years now?
Or is it mandatory to state the Name Change as Yes and provide a notarized Name change affidavit?
