new OCI for minor - documents checklist

Hi- Sorry if this is a repeat question but considering VFS site is fuzzy in terms of document checklist - I want to ask this again.
Is the below list of documents sufficient for Minor's new OCI card application , born in USA, to parents who are Indian Citizens.?
Also which of the following documents need to be notarized?

Copy of Minor's passport (Which pages??)
Copy of Both Parent's passport (Which Pages??)
Copy of Birth Certificate
Copy of Parent's Valid Visa (What if the latest Visa is expired??)
Copy of Parent' Marriage certificate
Photo
Minor signature (Is thumbprint required on white paper?)
Last page of govt. application
Parental Authorization Form
