Physical Application for OCI

The OCI checklist mentions affixing a physical photo to the physical application. Where can the physical application be found? I only have a print-out of the online government application and a checklist of documents. Thanks for your help!

The printout of the govt filed out application form IS the physical document,

Thank you- they also state that the signature should match the physical signature on the physical application. Do I attach the photo to the printout of the govt filled application and sign under it?

Just paste one photo on top of the printed copy of the photo and no need to sign under . Sign only the declaration page. Send one photo extra.
