Visa application - uploading documents and old visa type

I am filling application for my wife who is going to India for a medical emergency. We have filled out the application and now am on verification page. I have couple of questions.

1. What do you put for old visa type? last time wife went to India she went on Indian passport. Right now she put indian passport details in that section and chose visit visa since it was not letting her finish the application otherwise and indian passport was not an option.

2. she was not given prompt to upload any supporting documents such as copy of passport or copy of letter from Indian hospital. she was just asked to upload her photo. will she be asked to upload supporting documents after verification?

Reach out to the consulate for guidance on this, these visa systems are fairly new and I think there are bugs in the process still.
