Surrender Certificate - Please help

This is my husband's situation:

He applied for an American passport book and card. we received the passport book but the card and naturalization certificate did not come. When we called they said that they are trying to locate my husband's certificate.

I have the following questions:

1. Can he apply for the surrender certificate without Naturalization certificate
2. If no, can any other document be used as naturalization proof
3. do we have any other option?

He is travelling to India on the 4th of dec, and We need to apply for his surrender certificate and entry visa. Anyone knows anything... please help

Does he have a copy of Naturalization Certificiate? If he does, a self attested copy submitted with Surrender Certificate application should be acceptable.
