I have seen 2 versions of the minor consent form that needs to be notarized.
1. Consent form that says 'do hereby authorize the issuance of Renunciation Certificate/Declaration/OCI card for the said minor
2. Consent form that says 'do hereby authorize the issuance of OCI'
Can someone who has applied recently successfully, please confirm which form they have used
Also have they been successfully if the notarized form, has not been completed electronically.
